Participants’ in this program will enhance their ability to identify candidate for the AFC Program will prove a significant benefit to their clients as well as the general public. Many potential candidates are unaware of the availability of AFC program to help caregivers provide an at home situation for clients and avoid long term care facilities. By providing information to clients the opportunity to decrease medical treatments (hospitalization/illness) through RN and case management services and increase quality of life is provided to clients.

Relevant Experience: Mary Gill holds a Masters Degree in Mental Health Counseling and has provided training and education in the field of Human Services for 10 years. Provides AFC Overview to medical and health care professionals on a weekly basis (up to 4 hours per week.)
