Dear Family member and supporter,

The 2011 Ounce of Prevention Conference Planning Committee is now accepting nominations for regional Community Transformation Awards to recognize individuals, organizations, programs or partnerships/coalitions from each of the six Executive Office of Health and Human Services (EOHHS) regions. The awards will acknowledge and celebrate success stories related to the principal themes of the conference as presented in the three conference plenary sessions:

• Supporting Locally Grown Food: Synergy and Conflict
• Prevention in an Era of Healthcare Reform
• Making Us acCOUNTable: Data in the Service of Communities

WE would appreciate your help in nominating Dodge Park Rest Home to the Ounce of Prevention 2011.
